What is Testosterone Therapy?

Testosterone is the primary male sex hormone. It is produced by the Leydig cells in the testes in response to luteinizing hormone. Age related alterations in testosterone occur by two mechanisms. The primary mechanism is a decrease in the number of Leydig cells.

Neaves demonstrated that total number of Leydig cells is reduced by 44 percent in men over the age of 50. His study demonstrated that age was the single largest factor contributing to decline in testicular function (Neaves, Johnson, & Porter, 1994).

A secondary cause of dysfunction is failure of the hypothalamic-pituitary-gonadal axis. Declines in T levels are linear and continuous unlike the rapid fall in estrogen levels experience by menopausal females.


The linear nature of the condition ensures that not all men will experience andropause. Higher levels of T in early life may provide for less pronounced age-associated changes.
